The indicators presented measure the percentage of emergency admissions to any hospital in England occurring within 30 days of the last, previous discharge from hospital backdated to 2014/15.
The Adult Social Care Outcomes Framework (ASCOF) measures how well care and support services achieve the outcomes that matter most to people. The ASCOF is used both locally and nationally to set priorities for care and support, measure progress and strengthen transparency and accountability.

Number of computer-generated fit notes (eMed3) and fit note episodes at Sub Integrated Care Board Location level. Includes details of diagnosis (ICD10 chapter), duration, advice given, number of episodes and incidence of duplicates. Data are aggregated by month and published quarterly.
