View version history and restore previous versions of scanned paper notes

Find out how to view the version history and restore previous versions of scanned paper notes.

A new version of a patient’s scanned paper notes is created whenever you, or another practice: 

  • add pages to them 
  • remove pages from them (for example, the pages are for a different patient and have been removed and reassigned) 

The latest version of the patient’s scanned paper notes is listed in their Lloyd George records page, with the version number, for example ‘Scanned paper notes V2’. If no version number is listed, there are no previous versions of the scanned paper notes.  

You can view previous versions of the notes.


View previous versions of scanned paper notes

1. Go to the patient’s ‘Lloyd George record’ page by searching using their NHS number.

2. Select View next to the scanned paper notes.

3. Select View version history and restore

4. The current and previous versions of scanned paper notes are shown in date order, with the current at the top of the page and earlier versions below. You can select to view or restore previous versions of scanned paper notes.  

5. Select View to see a previous version of scanned paper notes.


Restore a previous version of scanned paper notes

You can restore a previous version of scanned paper notes from the ‘View version history for scanned paper notes page’, or when you view a previous version of the notes.  

You might want to restore to a previous version of the scanned paper notes if: 

  • you reassigned pages within the scanned paper notes to another patient by mistake  
  • you removed pages from the scanned paper notes to print and send to Primary Care Support England (PCSE) by mistake  
  • you added pages to the scanned paper notes by mistake  

You cannot restore a version of scanned paper notes if you remove the entire document from storage in the service.  

Before restoring a version, check how many pages it contains using the PDF viewer. Compare this to the other versions. If pages were added or removed in the versions in-between, they may not appear in the new version when you restore. Checking the page numbers and previewing the record helps you restore the right version and avoid losing information.

To restore a version of scanned paper notes: 

1. In the View version history for scanned paper notes page, select Restore version next to the version you want to restore.

2. You’ll be asked ‘Are you sure you want to restore this version of these scanned paper notes?’

3. If you want to continue, select ‘Yes, restore this version’ then select Continue to restore the version.


After you’ve restored a version of scanned paper notes

The version you’ve restored will be given a new version number. This version will be listed as the current version of the scanned paper notes in the patient’s Lloyd George record page.  

If you’ve restored a version by mistake, you can go to the View version history and restore page and restore the notes back to the previous version.
