Download electronic health record (EHR) notes and attachments from EMIS for upload to the National Document Repository

When to follow this guidance

There may be times when a patient’s electronic health record cannot be transferred or stored electronically, for example, where GP2GP fails or can’t be used, or if the patient does not have an active registration.    

To transfer or store the EHR without the need to print and transport it, you can export the patient’s documents from EMIS and upload them to the National Document Repository (NDR) Access and store digital patient documents service. Primary Care Support England (PCSE) or the patient’s new practice can access the record in the service, helping to provide continuity of care, whilst keeping patient information secure and accessible. 

Choose the option that matches what you need to upload:


Export a full summary

  1. Open the patient’s Consultations.
  2. Select Print.
  3. Choose Full summary.
  4. Select Print.
  5. In the print dialogue, choose Print to PDF.
  6. Create a folder named with the patient’s NHS number (optional, but recommended).
  7. Select Print to save the PDF.  
  8. Enter a filename and choose a place to save the document.

Export attachments

  1. In the top menu bar, select Documents.
  2. Make sure List documents is selected.
  3. Select all documents in the list.
  4. Select Export (from the right‑click menu or the top menu).
  5. Save the files in the same folder as the summary (if you created one).
  6. Wait for the export to finish — EMIS does not show a confirmation screen.  

The exported files are saved in your chosen location.


After you export the files

  1. Save all files in line with your organisation’s information governance policies.
  2. Upload the exported documents to the NDR Access and store digital patient documents service using the uploading guidance
  3. Check that all relevant documents are included before you upload them.
