Summary

The National Diabetes Inpatient Safety Audit (NDISA) is a subset of the National Diabetes Audit (NDA). It captures harms that can occur to inpatients with diabetes and seeks to measure the standards of care for these patients provided by NHS acute and community healthcare organisations in England and Wales, as assessed against the Getting it Right First Time (GIRFT) standards.

The NDISA harms collection is a mandatory year-round collection of 4 harms that can occur to inpatients with diabetes in acute hospitals in England. The data collection started on 1 May 2018 and was previously known as National Diabetes Inpatient Audit (NaDIA) - Harms (see Related Links).

The NDISA dashboard covers submissions of diabetes-specific inpatient harms in England and Wales between November 2024 and October 2025 whereas the detailed analysis file covers submissions between November 2018 and October 2025.

This is the third National Diabetes Inpatient Safety Audit (NDISA) release and contains a detailed analysis file and a dashboard.

Detailed Analysis File

The NDISA 2024-25 detailed analysis file release contains information from the inpatient harms collected in NDISA between November 2018 and October 2025.

It provides a review of the rates and risk factors for serious diabetes-specific inpatient harms in England only, using the NDISA harms collection.

Results are split into 3 sections:

  1. NDISA: Inpatient harms participation
  2. NDISA: Inpatient harms counts
  3. NDISA: Inpatient harms rates

Sections 1 to 3 use NDISA harms data linked to the core NDA and Hospital Episode Statistics (HES).

Dashboard

Dashboard

The dashboard includes the monthly submission rates for providers and sites for serious diabetes-specific inpatient harms in England and Wales, using the NDISA harms collection. These are:

  • Severe inpatient hypoglycaemia
  • Diabetic ketoacidosis (DKA)
  • Hyperglycaemic hyperosmolar state (HHS)
  • Diabetic foot ulcers.

Changes to NDA reporting and ISS survey

NHS England are currently reviewing the routine production of NDA State of the Nation reports. Please note that data will still be released via dashboards and standalone data files whilst this review is being conducted.

To help inform this review we would be grateful if users can provide feedback on their use of the State of the Nation reports using the feedback survey available in the ‘Related links' section of this page.

Additionally, the Integrated Specialist Services (ISS) survey is currently under review and will not proceed until further notice. Therefore, this publication does not include any data relating to the ISS survey.  Data from previous publications of the ISS survey can be accessed via the 'Related links' section of this page.
