New updates to the paper form for GP registrations

This update reflects several recent changes made to the online patient registration form that are designed to improve patient data collection and support national health initiatives.

In this new iteration of the form, there will be a question about past blood transfusions to support the drive to address the Infected Blood Inquiry. There will also be updated information on the Summary Care Record (SCR) and the basis of how that information is used for direct care.

While paper registration forms remain a useful option for individuals who may have limited digital access or confidence, online forms can offer a more streamlined and user-friendly experience for both patients and practices.


Key updates to the form

Infected Blood Inquiry (IBI) screen questions 

In support of the national drive to identify undiagnosed patients affected by infected blood, the updated form will include screening questions to help find individuals who may have been impacted.

These questions are a direct response to the IBI recommendations as part of a wider effort to ensure appropriate follow-up and care. The new question on the form mirrors the changes in NHS Online Registration service and asks patients born before 1996:

The introduction of this question provides an important prompt for GPs to make further enquiries and test where appropriate for hepatitis C as part of the new patient health check.

Summary care record (SCR) updates

The new paper form will also reflect recent changes to how GP registrations capture preferences for sharing of Summary Care Records.

It explains the basis on which information is shared for the purposes of direct care, and how the user can change their preferences using the SCR patient preference form.

These updates, outlined below, aim to improve understanding and transparency around how patient information is shared.

What practices need to do

We are asking for your support in rolling out the new paper form in your practice. Kindly take the following steps:

  • Ensure staff are aware of the new form and its contents.
  • Familiarise your team with the new questions to support accurate data entry.
  • Start using the new paper registration form at the earliest convenience. No need to throw old forms in the bin if they’re already printed. Start using the new paper form once these are finished.
