Additional armed forces question added to online GP registration form

The Register with a GP surgery online service has been updated to better identify and former armed service members. Find out more about the changes to questions we ask about patients who serve or have served in the Armed Forces.

As part of our ongoing commitment to improving patient data collection and access to health and care services, the online GP registration form has been updated to better identify and former armed service members. 

Historically, the form has included a single question aimed at identifying patients transferring from Defence Medical Services (DMS) GPs. This helps practices understand from where to request patient medical records.  

The online registration process now includes an additional question about armed forces service. This change is designed to ensure more accurate data collection and improved access to NHS priority services, like mental and physical health support and prosthetics.  


The new question

The online registration form now asks:  

  1. Have you ever been in the UK Armed Forces or Armed Forces Reserves? 

  1. Has a family member ever been in the UK Armed Forces or Armed Forces Reserves?

If a patient answers ‘yes’ to the first question, the second question is displayed. The patient registration email that GP practices receive will now reflect these changes and provide that additional information. See how the questions will appear in the online registration form below.

Why we are making this change

  • Improved identification: Practices can now more reliably identify veterans and their families, ensuring they receive the support and services to which they are entitled.  

  • Enhanced patient experience: By capturing this information at registration, practices can proactively offer tailored support, and signpost to relevant services.  


What this means for your practice

As with all changes to the online registration form, it is important to:  

  • Ensure staff are aware of the new questions in the online registration form and the information to expect.  

  • Signpost patients with armed service members to relevant specialist services.
