Direct Care APIs use case submission form

We need to understand how you plan to use the Direct Care APIs (GP Connect), or what problem you’re hoping to solve by using one of our products. This form is to gather that information and understand how this, in turn, will benefit the NHS and patients. 

Once submitted, the form will be reviewed by the team and a response provided within 14 days. If your use case is accepted, we will provide you with the relevant information to begin the onboarding process.

By completing this form, you are agreeing that your use case is for the purpose of direct care only.

Before you start

Before you start this form, you can read summaries of our APIs to help you decide on your preferred API. You should also visit the GP Connect prerequisites page on our Consumer Support Hub to make sure you understand the requirements of developing against the Direct Care APIs.

The National Data Sharing Arrangement (NDSA) sets out the data sharing requirements and obligations for the use of GP Connect for end-user organisations.

Your use case will not be reviewed until this has been received. Failure to send will result in your use case being rejected.

Section 3: Clinical safety prerequisites
Confirm if the following prerequisites have been met:

A qualified Clinical Safety Officer is in place *
A clinical risk management process is in place that is compliant with DCB0129 *
A clinical safety case and hazard log will be established for this product *

Section 4: Information governance (IG) prerequisites
Confirm if you are committed to the following:

A UK location for processing or storing patient data *
A cycle of penetration testing of your product within the last 12 months and on an annual basis at a minimum *
Returning the appropriate level of end-user information to be able to meet the GP Connect IG model *
