Cohort information

Information on the form should include:

  • a description of what you want to achieve, who you need to identify, and the goals or outcomes

  • the data sets you will need to use

  • how often you will need cohort data and what it will be used for

  • your timelines, including any critical deadlines

  • links to any relevant policies or guidelines
