Submission guidance documents

These documents support services and organisations to configure their systems to extract the data required for the AACC data set.

NHS AACC Data Set User Guidance

The NHS AACC Data Set User Guidance explains how the national data set should be used and interpreted by users, system suppliers and other stakeholders.

This document supports and should be read in conjunction with the Technical Output Specification and data model.

AACC Enhanced Technical Output Specification

The Enhanced Technical Output Specification (ETOS) expands upon the Technical Output Specification (TOS) that was provided as part of the Information Standard.

As well as input data items, the ETOS specifies how items are validated on submission, and details of the derived fields that NHS England provide as part of the output data set.

October 2025

The format definition for C102100 SUBJECTIVE CODE is stated in the specification as an 8. We are aware that new 9 digit codes are available from 1 October 2025. These will be accepted in submissions, despite the 8 digit limitation shown in the specification. The specification will be updated at a later date to reflect this change

AACC data model

The AACC data model shows how items and groups defined in the TOS relate to each other in an entity relationship diagram style format.

AACC XML input and output schemas

AACC input and output XML schemas are available on the TRUD website.

The XML extract schemas describe the structure of the exports generated by NHS England and can be used to assist in development of in-house analytical and checking tools.

The “input” schema specifies the items and group structure required for submissions sent to NHS England. The “output” schema specifies the items and group structure, including derived items as processed by NHS England.

AACC Item completion guide

This guide provides a visual cross-reference of expected item completion for each service request type (for example CHC checklist or direct referral) within the AACC101 referrals assessments and outcomes group.

AACC glossary

Terms used throughout the All Age Continuing Care data set are not available in the NHS Data Dictionary, as terminology is based on the national frameworks.

Please refer to the AACC glossary for definitions of terms used.
